Can adult health and social care still have involvement in giving information in support options when someone has continuing health funding? My knowledge was that it was the role of CHF care manager to be responsible for giving the options of support and adult health and social care were no longer involved once someone was assessed for continuing health funding.
Any advice would be much appreciated.

Maybe give us a bit more age about the age of the patient and what is happening?
