Support bubble outside Tier 4

I’m caring for my wife, who’s recovering from major surgery and is ‘clinically extremely vulnerable’, and our twin, 2-year old toddlers. We live in London. We currently receive weekday support from a nanny, however, she is now away until the new year. It had been our intention to stay with my wife’s parents in the Cotswolds - as both of us have been shielding - and this would have allowed me to continue working remotely. However, I’m concerned that we are now prohibited from doing so, by the Tier 4 restrictions. Any advice?
