Care needs assessment

Dear all,

Mum is currently in a nursing home, placed there after a short hospital admission. Prior to that she was at home with a myriad of different carers. This admission I see as the crisis that was always going to happen. Next week we begin the process that is planning for mums care going forward. A visit has been booked with the social worker. How do I prepare for this so that I can achieve the best outcome for mum. I would really welcome your thoughts on this from your experiences.

Before a social worker gets involved, mum should have an NHS Continuing Healthcare Assessment, to see if she is entitled to entirely free care. Ask Google for full information.
