We have been kind of left dangling in the wind for years.

Then think of this as a “New Beginning”.
As dad came out of hospital and went back in again, I would start by complaining to the CEO of the hospital.
The only way I ever got help from a hospital, almost instantly!

So ring up, ask to speak to the CEO or his PA.
Be sure to use the words “unsafe discharge”, the equivalent of an alarm bell.
Insist that he has a Continuing Healthcare Assessment before discharge is even contemplated, and that MUST include you.
Dad should have been given a booklet about his discharge, on admission. I saw one on the internet yesterday, but couldn’t save it on my tablet. It was a .gov document so you should be able to find it.
Insist that if he isn’t immediately entitled to Continuing Healthcare (CHC) then he MUST have his 6 weeks free care this time, and they must arrange proper assessments from Social Services, for both you and dad.
Also be sure to use the words “I cannot get out at all” and “desperately need help”.

I know it’s a very difficult subject, but has anyone talked to you about dad’s life expectancy?
